Starting the VFY Grant Process

Four Important Issues
There are four influential issues to granting a support gift. Each issue builds on the next issue of concern to help create an extraordinary understanding that will help a family going through a major illness. A gift experience is frequently a source of inspiration for a patient and their family undergoing difficult medical treatments. Our grants of certain gifts are to help build a positive reinforcement that helps a family overcome one or more of their obstacles. A grant experience is a means of getting things done when someone does not have the financial means or resources on their own to accomplish doing it on their own. It’s our desire that they see VFY as a means that rekindles their belief in themselves and that people do care about what happens in their life and family.

Step 1: Referral
VFY relies on medical professionals, and social workers for recommendations. Our desire is to work with adults and children who have reached the age of 3 ½ and are under the age of 39 at the time of referral recommendations who have not received a wish or grant from another wish-granting organization.

Step 2: Medical eligibility
VFY’s volunteer staff will determine a person’s medical eligibility with the help of the treating physician or hospital social worker. To receive a grant, the person must be diagnosed with a life-threatening medical condition – i.e., a progressive, degenerative or malignant condition that has placed the person’s life at risk.

Step 3: Finding the real need
Because our resources are limited, VFY will use one or more of our passionate evaluator team members to investigate what we can affordably do. In many cases, our phone interview will determine if an applicant meets the qualifications. These loving volunteers connect with patient and family and help to explore what is needed now and down the road and will help the family to explore what additional options they might have.
 
Step 4: Creating the Grant Platform
Our evaluation grantors will try to create resources and/or a grant package that both the family and the VFY grant team will work on together creatively to determine what is going to be needed now for immediate needs and what may be needed in the long term future. Jointly we will try to come up with a solution for grants to help the family work through the medical challenge in their own community.

Grants
Individuals cannot directly apply to VFY. Only individuals living and receiving treatment in the Northern California region are eligible. The process is by nomination only through a network of social workers at participating hospitals.

The VFY grant program is for young people who are documented medical patients referred by a medical social worker or their medical practitioner.

The grants are designed to assist an individual child or young adult parent of children under the age of eighteen undergoing medical treatments.

Day Care Grants
Awarded applicants must submit required information on a licensed day care facility to qualify for a day care grant. Medical patients must be under the age of 39 with young children who need a wider support group outside of family and friends.

The applicant must demonstrate that they need help in their family and need extra income or day care while they undergo daily or weekly medical treatment. Grants will range from $500 up to $5,000 and support day care costs while a patient is in treatment. The grant application must give an estimated breakdown of related expenses by a medical practitioner or social worker of an eligible county or medical treatment center.

Breast Reconstruction Plastic Surgery Grant
In certain insurance cases where elective surgery is done on a breast and emergency treatment is done an infected breast, and not fully covered, applicants may apply for an assistance grant to help with medical cost or reconstruction Plastic Surgery. Applicants must also supply an oncologist medical report, surgeon report and the plastic surgeon’s certification and recommendations.
Once submitted, a funding committee will notify the applicant of the committee’s agreement to fund a grant or deny. Funding is based upon funds available and in order of date received. Applicants are notified within 30 days and funds will awarded each quarter to cover some or all of the plastic surgery reconstruction expense. Grants will range from $1,000 to $5,000.

Medication and Deductible Grant
VFY will help in providing resources for medical prescriptions and medical deductions. Awarded applicants can apply for documented estimated medical expenses ranging from $1,000 to $5,000. Funding is based upon funds available at the time of request.
